Helping you with your day-to-day needs is the focal point of care from OrthoFit. With this in mind, we understand the power of good information. It is good to know that Medicare coverage provides for one (1) pair of diabetic shoes and three (3) sets of diabetic inserts per calendar year as medically necessary. Medicare usually covers 80% of the cost. Some secondary insurance companies may cover part or all of the remaining 20%.

Please follow the instructions below if you wish to determine if the cost of your shoes, inserts, or modifications might be covered by Medicare. Failure to follow the instructions may cause Medicare to deny your claim.

Before Your First Fitting Appointment

Before you arrive for your fitting appointment, obtain a prescription from your physician (MD/DO) for diabetic footwear. A diabetes assessment or foot exam must have been performed by a MD/DO within six (6) months of shoe fitting indicating that you have one or more of the following qualifying conditions:

History of partial or complete amputation of the foot

Z

History of previous foot ulceration

Z

History of pre-ulcerative callus

Z

Peripheral neuropathy with evidence of callus formation

Z

Foot Deformity

Z

Poor Circulation

What to Bring to Your Appointment

Please bring the following with you to your appointment:

Prescription for diabetic shoes from your physician (MD/DO)

Z

Report from the most recent diabetes foot exam / assessment

Z

Proof of insurance (insurance cards)

Z

Photo identification

If you have ALL of the above information completed prior to your visit, we may be able to process your prescription. If you have any questions, please feel free to contact us.
